HSHS Sacred Heart Hospital to offer advance directive seminar June 4


The Volunteer Partners of HSHS Sacred Heart Hospital are hosting a free advance directive seminar for community members to learn about the importance of having an advance directive – from both a legal and ethical point of view. An advance directive is a legal document that names a person to make health care decisions if an individual is unable to make decisions personally.

The seminar will take place Monday, June 4, 6 p.m. until 7:30 p.m., at Sacred Heart Hospital in the Monsignor Klimek Auditorium (900 West Clairemont Ave., lower level).

An advance directive can be filled out at the seminar. If you plan to attend, please bring the contact information – name, address and phone number – of two people to be designated as your decision-making agents. The Volunteer Partners will have witnesses available to sign the documents.
