You can play an active role in insuring your visit is a safe one by carrying a List for Life. A List for Life is a record of any chronic conditions you may suffer from. Your allergies, the medications and supplements you take regularly and who to contact in case of emergency.

Because mixing or confusing medications can be fatal, keeping an updated list in a purse or wallet may save your life. For your convenience, we have provided a List for Life that you can print and update regularly using a pencil for easy erasing. Make sure you update your list of medications regularly from every medical professional you visit. Take your list whenever you visit the hospital, doctor, dentist or pharmacy. This list can also be a lifesaver in an emergency if you are unable to communicate.
