  • You can play an active role in insuring your visit is a safe one by carrying a List for Life. This is a written list of any chronic conditions you may suffer from.
  • It shows your allergies, the medications and supplements you take regularly.
  • It says who to contact in case of emergency.

Why it’s important—for life

Because mixing or confusing medications can be fatal, keeping an updated list in a purse or wallet may save your life—especially if you’re in an emergency and unable to communicate.

For your convenience, we have provided a List for Life that you can:

  • Print and update regularly using a pencil for easy erasing.
  • Make sure you update your list of medications regularly from every medical professional you visit.

Take along whenever you visit the hospital, doctor, dentist or pharmacy.
