Arrival and Discharge

 

Admissions

Your physician (a member of Sacred Heart Hospital’s medical staff) will arrange for your admission to the hospital. When you arrive at Sacred Heart Hospital you will be registered in our Admitting Office. All the information we request is necessary for your patient records and for processing insurance forms.


Pre-registration
The Admitting Office has a pre-registration program for your convenience. Whether you are being admitted to the hospital or here for an outpatient service, pre-registration expedites the time you need to spend in our admitting office. To pre-register, please call 715.717.3906 from 8 am to 4 pm.

 

Your Care Team
Your physical, psychological, social and spiritual well-being are the primary concern of the team of dedicated healthcare professionals who will care for you at Sacred Heart Hospital. Members of your care team may include physicians, nurses, social workers, therapists, dieticians, pharmacists and those in pastoral care.

This team will work together throughout your stay to assess your needs and provide you with the care and information you need to return to health. If you experience any pain as a result of your illness, injury or procedure, please tell your nurse. Relief of pain is part of our healing mission and your comfort is important to us.

 

Personal Belongings/Valuables and Money

Because there are no facilities in our patient care units to secure valuables (such as large sums of money, credit cards, jewelry, cameras and items of sentimental value) we ask that you please send these items home with your family or friends.

We cannot be responsible for valuables kept in your room. If you are unable to send your valuables home, we can deposit them in our “Safekeep” safe where they will remain until you are discharged. Please ask your nurse if you need this service.

You May Want to Bring These Items
Although we will provide for most of your needs during your stay, you may want to bring the following items for your comfort:

  • Your insurance, Social Security, Medicare or Medicaid cards.
  • A robe, slippers, and sleepwear.
  • Toiletry articles such as a comb, toothpaste, toothbrush, shampoo and deodorant if you prefer not to use those supplied by the hospital.
  • Because of safety regulations, we ask that you not use electronic appliances such as hair dryers, razors or radios from home unless they are inspected and approved by our hospital's security department

 

Accommodations
Some hospital departments offer a choice of private or semi-private rooms. We will make every attempt to honor your preference.

Our fifth and sixth floors have been remodeled to better meet your needs. Each room is private and has a private bathroom and shower. We invite you to take a virtual tour of these new rooms.

If you desire a private room, make your request known to our registration staff upon admission. If you are not satisfied with your room, consult with the department director. He or she can arrange your transfer when a space becomes available.

 

Medications
All the medications you will need during your stay are prescribed by your doctor and provided through our hospital pharmacy. If you brought any medication with you, give them to the nurse so that they can be identified. Those medications should be sent home with a family member or friend, or they can be stored at our hospital and returned to you upon discharge.

Care and Safety Concerns
Our intention is to provide excellent care for you. If you have concerns about your care and/or safety, or that of a loved one, while at Sacred Heart Hospital, please ask your caregiver to speak with the department director or the patient services house supervisor on call. Discharge Planning
The discharge planning process is focused on your continued care needs. It is important for you to understand your medical condition and begin on admission or soon after to participate and plan for continuing care before you are discharged. This process combines the expert knowledge and experience of your entire care team. They will help you and your family identify any health and wellness needs you may have after your hospital stay.

Day of Discharge
When you doctor determines that you no longer need specialized inpatient hospital services, he or she will issue discharge instructions and orders. Your nurse will review the instructions with you so that you understand your continuing care needs. Arrangements will also be made to assist you to your vehicle or your ride home.

Case Management

Sacred Heart Hospital offers supportive counseling and discharge planning services to you and/or your family member. These services are provided by case managers and social workers and may include: arranging services such as home healthcare, nursing home placement, rehabilitation placement and other community aid to meet your continuing care needs after discharge. For information or assistance, call the hospital’s main number at 715.717.4121, and ask to speak with a case manager or social worker. On weekends, please call our pager at 715.717.4195. Business hours are Monday through Friday, 8 am to 4:30 pm. Saturday 8 am to 4pm and Sunday 8 am to Noon.

 

Home Health and Hospice Services

Your care team will help you look at all the options available for assistance at home following discharge. To assure that you receive the assistance you need arrangements can be made for supportive help, home care nursing, therapy or hospice care.
